Clinical Analysis of 1,000 Cases Pelviscopic Surgery in Gyecology


Abstract
This study is about clinical evaluation of 1000 cases who were admitted and treated by pelviscopic operation at department of Obstetrics and Gynecology Chung Ang University Yong San Hospital from August, 1989 to July, 1993.
Today endoscopy has revolutionized the practice of gynecology.
It has modified the thinking approach, diagnostic ability, and operative capability of gynecologists involved in techniques of pelviscopy.
Operative laparoscopy (pelviscopy) has reduced the need to perform laparotomy, and as a result has decreased patient discomfort, length of hospital stay, and work disability, and possible, increased the frequency of out patient surgical treatment of gynecologic disease, formerly requiring laparotomy and hospitalization.
Modern methods of operative pelviscopy have been successfully applied to the treatment of gynecologic disease, in this report, the author presents his experience with the diagnosis and management of the pelviscopic surgery.
The results were as follows
1) The majority of patients were between 20 and 40yrs of age. There was no significant difference in parity.
2) The previous operation history was found 15.8% (158 cases), operation were represent to c-section in 32.9%, ectopic pregnancy in 222%. oophorectomy in 13.3%, appendectomy in 7.5%.
3) Preoperative indications were myoma 325 cases(32.8 % ). adnexal mass 330 cases(33 % ), ectopic pregnacy 132 cases(13.2 %) chronic pelvic pain 112 cases (11.2%) diagnostic infertility 94 cases(9.4% ), and etc.
4) The majority pelviscopic surgery were PAVH(298), salping oophorectomy(172) oophorectomy(150), incidental appendectomy(190) and etc.
5) The common complications were subcutaneous emphysema 12 cases(52%) preperitoneal bleeding 5 cases(22.2% ), bladder injury 5 cases (22.2%), bowel injury 4 cases (17.3%), and peroneal nerve palsy 1 case.
Pelviscopic procedure well tolerated by most woman generally providing rapid recovery, cosmetic. hospital stay than laparotomy.
